As a private Medical Practitioner, my prime responsibility is to the welfare of my patients which encompasses an ethical responsibility to maintain confidentiality and protect my patients’ privacy. This practice values the doctor-patient-relationship and privacy is vital to such a relationship.

At this practice we collect personal information from our patients for the purpose of providing health care services. The provision of these services includes the obtaining of pathology, radiology and other related services.

The confidentiality of personal information held at this practice remains of paramount concern. Our staff are all aware of the importance of patient confidentiality. We will only use or disclose the personal information of our patients for the purpose of providing health services, for limited related purposes, or where required to by law. Doctors in this practice are members of medical defence organisations and may need to disclose some personal information to these organisations from time to time. In other situations we do not disclose personal information without obtaining consent.

We endeavour to ensure that all personal information collected by this practice is accurate, complete and correct. This is updated regularly as ongoing treatment is updated per general practitioner referrals on a 12 monthly basis.

If you wish to access your personal information held at this practice please collect and complete a copy of the Request for Access form available at reception. Should access be denied, a reason for this denial will be provided to you. (If you require a copy of your personal information, a fee to cover costs is charged for the service).

To ensure the security of personal information held in this practice all records are stored in filing cabinets and the premises are under 24 hour security surveillance. Information stored on computer is password protected and only accessible by authorised staff in the practice.
This practice will only use the Medicare numbers collected from patients for the purpose of billing for medical services provided to its patients. Similarly we would not use or disclose any other government identifying number which you may give to us.

Should it become necessary to transmit your personal information overseas we will endeavour to ensure that your personal information is adequately protected.

Should a patient wish to complain about our medical services or any other related matter they should contact the practice manager, Jennifer Stokes in the first instance. The matter will then be discussed with the doctor and staff concerned and a written response will be forwarded to you.
